Please provide the information for all the trip request boxes especially the Additional info/situation box. A photo will also be helpful as well as whether a crate will be provide or not, and if so, what size LxWxH. Contact information (cell phone and email address) for sender and receiver should also be provided. For 501c3 organizations, provide the EIN.
